Upload Your COVID Vaccination Card Instructions

As we prepare for a fully open and operational 2021-2022 academic year, consistent with state and federal guidance, all undergraduate, graduate students and employees who will be on campus after August 1st will be required to be fully vaccinated against COVID-19.
The more information the University has regarding who is vaccinated, the better understanding we have of the degree of protection present in our campus community. This is the best way to guarantee our plans for a near-normal environment with full in-person learning, at-capacity residence halls, and in-person student engagement opportunities.
If you have already been vaccinated, congratulations and thank you!
If you have not, you may search for a vaccine location using the COVID-19 Vaccine Finder. For those who live outside of Massachusetts, please refer to your state’s Covid-19 Portal.
Once you are fully vaccinated, please provide Bay Path with proof of your COVID-19 vaccination by visiting the Bay Path portal and sign in with your Bay Path User ID and Password. There you will find easy-to-follow instructions on how to securely upload your vaccination card.
Completion of your daily symptom check will be required until further notice. Should you have any questions, please contact the Campus Public Safety Department at 413.565.1224.
