COVID Vaccine Mandate Reminder

Vaccine Mandate

All undergraduate and graduate students, and employees who will be on campus on or after August 1st will be required to be fully vaccinated against COVID-19.

Once fully vaccinated, students and employees shall provide Bay Path with proof of their COVID-19 vaccination.

Employee Exemptions

Employees who decline the vaccination must provide a Medical or Religious Exemption Form no later than July 26, 2021. Completed medical exemption forms should be sent directly to Deb Baker, Director of Health Services at dbaker@baypath.edu.

Religious exemption forms, once completed by the requestor, will automatically be forwarded to Javier Padilla, Assistant Vice President and Director of Human Resources.

Medical and religious exemption forms can be found on the My Bay Path portal under the Welcome page.

Student Exemptions

Students who decline the vaccination must provide a Medical or Religious Exemption Form no later than July 26, 2021. All student medical exemption forms must be sent to Deb Baker, Director of Health Services at dbaker@baypath.edu.

All student religious exemption forms, once completed by the student, will automatically be forwarded to Anne Chapdelaine, Dean of Students.
